Requirements for Admission

Below is a list of the forms and documentation required for each applicant. In order for the Pontifical College Josephinum to consider an application, the following documents and forms must be completed and submitted for review by the Admissions Committee.

Admissions Committee

The Josephinum admissions committee will review each completed application promptly, and the rector/president will inform the applicant of the status of his application. Only candidates who meet the requirements for acceptance will be considered by the admissions committee for admission in the fall semester of that year.

College of Liberal Arts / Pre-Theology Program

Forms and requirements are listed here.

  1. College/Pre-Theology Application Form: This form can be downloaded and filled out, then submitted back to us online

  2. Non-refundable application fee of $125. This can be paid online

  3. College/Pre-Theology Canonical Suitability Profile Interview: performed with Vocation Director, then signed and submitted with Diocesean Seal. This form needs to be mailed in since we need the original Diocesean Seal.

  4. College/Pre-Theology Declaration of Freedom: performed with Vocation Director, signed and then submitted back to us online

  5. College/Pre-Theology Completed Psychological Assessment Procedures: filled out by testing psychologist; includes release form. This form is sent back to us by the testing psychologist.

  6. College/Pre-Theology Chronological Log Form: This form can be downloaded and filled out, then submitted back to us online

  7. College/Pre-Theology Autobiography: This form can be downloaded, signed, and uploaded along with a word document including your autobiography. Submit back to us online here.

  8. Letter of Sponsorship from (Arch)Diocese, Religious Superior or Vocation Director: sent via mail or submitted back to us online.

  9. Recommendation letter from your current pastor: sent via mail or submitted back to us online.

  10. Most recent Formation Report from any previous seminary or formation program: submit back to us online

  11. Applicants coming from High School: Recommendation letter from an instructor: submit  Submit back to us online.

  12. Applicants coming from Post-Secondary School: Recommendation letter from a professor or instructor: Submit back to us online

  13. Certificates of baptism and confirmation (issued in the previous six months with the raised parochial seal) need to be mailed in.

  14. Applicant’s Record Request Form: print out and send to previous school to fill out

  1. Hard copy of high school transcript with graduation date or GED scores, these need to be mailed in.

  2. Official ACT or SAT test results (not required for those with a BA degree)

  3. Official copies of all post-secondary transcripts

  4. Military education documentation, if applicable

  1. Certified copy of birth certificate issued by local county or state health department. Photocopies are not accepted. Please mail in.

  2. Copy of passport or driver’s license

  3. Criminal background check from state of residence (completed by the Josephinum)

  4. Copy of honorable military discharge or retirement, if applicable

  1. Recent photograph (headshot) of the candidate

  2. Results of a thorough and complete physical examination with an up-to-date listing of completed immunization and vaccination records

School of Theology

Forms and requirements are listed here.

  1. Theology Application Form: This form can be downloaded and filled out, then submitted back to us online.

  2. Non-refundable application fee of $125. This can be paid online.

  3. Theology Canonical Suitability Profile Interview: performed with Vocation Director, then signed and submitted

  4. Theology Declaration of Freedom: performed with Vocation Director, then signed and submitted with Diocesean Seal. This form needs to be mailed in since we need the original Diocesean Seal.

  5. Theology Psychological Assessment Procedures: filled out by testing psychologist; includes release form

  6. Theology Chronological Log Form: This form can be downloaded and filled out, then submitted back to us online

  7. Theology Autobiography Form: This form can be downloaded, signed, and uploaded along with a word document including your autobiography. Submit back to us online here.

  12. Certificates of Baptism and Confirmation from Parish with a raised Parish Seal within the last six months: This needs to be mailed in to us.

  13. Applicant’s Record Request Form: print out and send to previous school to fill out
